Nominations must be submitted, by fax or email utilizing the form below, to the Central Rhode Island Chamber of Commerce, 3288 Post Road, Warwick, Rhode Island.  All Nomination packages must be received by Wednesday, December 26, 2008 and must include a signed Disclosure of Information form.

Selection Procedures

A panel of judges, selected by the Central Rhode Island Chamber of Commerce, will review the nomination packages for each award and the winners determined.  Decisions of the judges are final.
